Thinking about moving from semaglutide to tirzepatide? You're in good company. In one survey of people starting tirzepatide, about one in eight had taken semaglutide before.Obesity PillarsSourceGibble TH, Makin H, et al. Maybe your progress has slowed, the side effects have been a nuisance, or your coverage changed. Whatever the reason, here's how a switch usually works, what to expect, and why it's a team effort with your clinician.

01The short answer

Can you switch from semaglutide to tirzepatide?

Yes. You'll need a new prescription from your clinician, who decides when to switch and which dose to start on.Eli Lilly and CompanySourceLilly Medical. Semaglutide is the medicine in Wegovy® and Ozempic®, and tirzepatide is in Zepbound® and Mounjaro®. They work in a similar way: semaglutide acts on the GLP-1 receptor, and tirzepatide on both the GLP-1 and GIP receptors.U.S. Food and Drug AdministrationSourceFDA prescribing information and Medication Guides for Wegovy® (injection and tablets), Ozempic®, Zepbound® and Mounjaro®: Indications and Usage; Dosage and Administration; Drug Interactions; Adverse Reactions; Clinical Pharmacology; Clinical Studies. All sources

02Why people switch

Why do people switch?

The reasons are usually practical:

  1. Hoping for more

On average, tirzepatide led to more weight loss in the one study that compared them, so some people try it after a long plateau. The New England Journal of MedicineRandomized trial · 2025SURMOUNT-5People lost 20.2% of their weight on tirzepatide and 13.7% on semaglutide, in the only trial to compare the two directly.751 people · 72 weeks. 2. Side effects

If semaglutide's side effects have been hard going, a different medicine may suit you better. Everyone's a little different.American Diabetes AssociationSourceAlmandoz JP, Lingvay I, Morales J, Campos C. 3. Cost or coverage

Plans change which medicines they cover, and cash prices differ, so a switch can follow your coverage.CVS HealthSourceCVS Health. CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options. May 28, 2026.Open the source All sources

03Before you switch

Should you switch, or stay on semaglutide?

It depends on how semaglutide has gone for you. In one review of people who switched, those who'd hit a plateau after losing weight on semaglutide lost about 8% more over six months. Those who hadn't responded much to semaglutide lost about 3%.ObesitySourceBarenbaum SR, Gonzalez A, et al. Real-world weight-loss outcomes in weight-reduced patients treated with tirzepatide. 04How it works

How does a switch usually go?

Here's the usual path, step by step:

  1. Talk it through

Your clinician looks at why you'd like to switch and whether it fits, then writes a new prescription.Annals of Internal MedicineSourceQaseem A, Cross JT, et al. Pharmacologic treatments with lifestyle modifications in nonpregnant adults with overweight or obesity in outpatient settings: a living clinical guideline from the American College of Physicians (April 2026). 2. Stop one, start the other

You don't take both. Many clinicians start tirzepatide when your next semaglutide dose would have been due, about a week after your last one.American Diabetes AssociationSourceAlmandoz JP, Lingvay I, Morales J, Campos C. 3. Start at a low dose

Like any new GLP-1, tirzepatide usually starts low. 4. Step up and check in

05Your dose

What dose of tirzepatide do you start on?

Your clinician decides, and there's no official conversion between the two. Lilly, which makes tirzepatide, says it can't give a dose equivalence and leaves the choice to your clinician.Eli Lilly and CompanySourceLilly Medical. 06Timing

How long do you wait between semaglutide and tirzepatide?

Usually not long. A common plan is to take your first tirzepatide dose on the day your next semaglutide dose was due.American Diabetes AssociationSourceAlmandoz JP, Lingvay I, Morales J, Campos C. 07What to expect

What does switching feel like?

A smiling man holding up a blue injection pen

Your stomach may need a few weeks to settle again, which is one reason most people start low. In one study of people with type 2 diabetes moving to tirzepatide, about one in eight noticed stomach side effects. Nearly all were mild.Endocrine PracticeSourceJabbour S, Paik JS, et al. Appetite control can feel a little lighter for the first few weeks on a starter dose. Weight loss may slow while you step back up, too. Two new habits to know. If you take birth control pills, tirzepatide's label suggests adding a backup or non-pill method for a while: four weeks after you start, and after each step up. And if you take insulin or a sulfonylurea, your clinician may lower those doses when you start.U.S. Food and Drug AdministrationSourceFDA prescribing information and Medication Guides for Wegovy® (injection and tablets), Ozempic®, Zepbound® and Mounjaro®: Indications and Usage; Dosage and Administration; Drug Interactions; Adverse Reactions; Clinical Pharmacology; Clinical Studies. All sources

09Cost

What does switching cost?

Paying cash, tirzepatide usually costs a little more. Zepbound® is $299 a month at 2.5 mg, $399 at 5 mg and $449 at higher doses when you refill within 45 days.Eli Lilly and CompanySourceLillyDirect. At the programs we rank, compounded tirzepatide costs about $40 to $100 a month more than compounded semaglutide. The good news: it's one price at every dose. We checked those prices in October 2026, and the program confirms yours at checkout.

If you've paid for several months up front, ask how your plan handles a change before you switch.

10Compounded

What if you're on compounded semaglutide?

One handy habit: compounded doses are often drawn up in units on a syringe, and units depend on the vial's strength. So your old number of units won't carry over.
